1. Must be an Australian citizen or a permanent resident of Australia
2. Must be 15 years of age, or older
3. Must be unemployed and actively seeking work
4. Must meet all other enrolment requirements

Required Application Forms

The following forms have to be completed:

1. Confirmation of Enrolment Application Form (PDF)
2. Application for Enrolment (PDF)
3. Referral Form to RTO (PDF) – (this is only necessary and MANDATORY if you are registered with an Employment Service Provider (ESP), which means you have a Jobseeker ID and MUST have Centrelink CRN number).
4. Literacy Test (PDF) - The literacy exercise must be undertaken and submitted with the forms above.

GUIDE to filling the Confirmation of Enrolment Application Form:-

This form MUST be properly completed. To assist you with the form the following IMPORTANT numbered sections have been further clarified below:-

Item Number 3: Productivity Places Program - Eligibility

You MUST supply at least ONE copy of the documents from the Residency category and at least ONE copy of the documents from the Age/Identity category. Please note that these MUST be CERTIFIED copies. (These documents should be certified at the same time when the Statutory Declaration is signed – see below Item Number 6)

Item Number 4: Applicant’s Circumstances

You must tick either in Section 1 OR Section 2. 

If you have ticked Section 1 you must fill in the “Referral to RTO” form and see your ESP to complete the form. It is important for you to enter your Jobseeker ID and CRN number

If you have ticked Section 2 and have a Centrelink statement or card please supply a copy of it with your application which displays your CRN number.

Item Number 6: Statutory Declaration

You MUST sign this part of the form in front of an Authorised Person. A list of people who are authorised is given on Page 5 of this application form. The same person MUST certify the documents in Item Number 3.
